Board Action

The Fonda-Fultonville Central School Safety Committee shall be appointed and charged by the Board of Education as required under section 155.4 of the Education Law as amended October 7, 1999.

Composition

1) The Health and Safety Committee is required to be “…comprised of representation from district officials, staff, bargaining units and parents.” The Health & Safety Committee shall consist of the following:
Superintendent, Assistant Superintendent or their designee
a) Coordinator of Health-Safety Risk Management Services
b) Representative of the Maintenance/Buildings & Grounds Department
c) Bargaining Unit Representative-Instructional
d) Bargaining Unit Representative-Non-Instructional
e) Parent Representative
2) During school construction project, the Health and Safety Committee “…shall be expanded to include the project architect, construction manager, and the contractors.”

Charges

1) Develop and implement, in consultation with and approval of the Board, the required annual Safety Rating System.
2) Designate at least one member to participate in the required Annual Visual Inspection.
3) Receive written complaints related to health & safety; forward such complaints to appropriate resources for investigation, inspections and/or testing; and participate in creating and documenting written responses to such complaints.
4) Be involved during school construction projects to monitor safety following procedures established by the Board.
5) Conduct a post-construction walk-through inspection to confirm that the area is ready to be reopened for use.

Procedures

1) Meetings
a) The Health & Safety Committee shall meet at least quarterly.
b) Additional meetings will be scheduled as necessary to address:
i) Increased activities during school construction projects.
ii) Complaints related to health and safety.
iii) Development of the annual Safety Rating System.
c) One or more individual members of the Health & Safety Committee may additionally be involved on a periodic basis with:
i) The Annual Visual Inspection.
ii) Activities to monitor Health & Safety during school construction projects.
iii) Post-construction walk-through inspections
d) The Health & Safety Committee shall provide agendas and minutes of such meetings to the District Superintendent and the HFM BOCES Health-Safety Risk Management Service.
e) Health and Safety Committee Meeting activities shall be limited to issues and actions involving health & safety. Other issues, such as, program, labor, administrative, etc., are not to be addressed at Committee meetings.
2) Safety Rating System
a) Procedures to be developed and implemented in consultation with the Board.
3) Annual Visual Inspections & Five Year Building Condition Surveys
a) The following Health & Safety Committee members will participate in the Annual Visual Inspections & Five Year Building Condition Surveys:
i) Coordinator of the Health-Safety Risk Management Service
ii) Superintendent of Buildings & Grounds
4) Complaints Related to Health & Safety
a) The Health & Safety Committee shall designate a contact person who shall receive complaints received in writing related to health & safety.
i) If the complaint is urgent and of serious nature, the contact person shall, after consultation with appropriate administrative contacts, determine if further investigation, inspection or testing may be necessary and forward to appropriate resources for evaluation. Details of actions taken shall be
ii) submitted at the next Health & Safety Committee meeting.
iii) If the complaint is not urgent or of serious nature, the contact person shall place the item on the agenda of the next Health and Safety Committee meeting.
b) The Committee shall notify building occupants that written complaints shall be forwarded to the designated committee member.
c) The Committee shall, after consultation with appropriate administrative contacts, determine if further investigation, inspection or testing may be necessary and forward to appropriate resources for evaluation.
i) If no further action is required, the Committee shall respond in writing to written complaints in a timely fashion under guidelines set for in section 155.4
(d)(7)(i)(a).
ii) If further action is required, the Committee shall respond in writing to written complaints in a timely fashion under guidelines set for in section 155.4 (d)(7)(i-iv).
5) Involvement During School Construction Projects
a) Procedures to be developed and implemented in consultation with the Board.
